3.5-story Federal-style residential building completed in 1823 as a house and carriage house for James Roosevelt, FDR's great grandfather. It was occupied by the Roosevelts until 1857, when it became the New York Infirmary for Women and Children, established by Elizabeth Blackwell - the country's first female medical doctor.

By 1891, the building had been repurposed as residential, with a feather store on the first floor, and the carriage house enlarged and converted to a restaurant. In 1903, the upper floors were converted to manufacturing lofts, with the ground floor occupied by a restaurant. In the 1960s, the building was re-converted to residential. The ground floor is currently occupied by Bleecker Street Bar.

The walls are clad in orange brick, painted red on the bottom and top floors. It has a peaked roof, with two dormers with triangular tops on the north and south sides, above metal cornices. The west elevation has two bays of windows; the northern window on the 3rd floor has been bricked-in. On the south end is a 1-story and 2-story extension. The 1-story section has a broad iron roof cornice with modillions and swag in the fascia board.
