4-story office building completed in 1914. Designed by Gillespie & Carrel, it is clad in brown brick above a limestone ground floor. Both main facades are four bays wide, with three windows divided by dark-brown iron mullions in three of the bays, and a narrower 2-window bay at the far ends. Each window is also divided vertically into three panes. The main entrance is in the south bay on the avenue, topped by a cornice carried on console brackets. The east bay on 39th has a service entrance with metal doors and another cornice with brackets. The other bays have metal-and-glass storefronts.

There are dark-brown metal paneled spandrels between the 2nd & 3rd floors in each bay, and stone band courses at the top of the 3rd floor and bottom of the 4th. Both facades are crowned by a black metal roof cornice with modillions and dentils. The ground floor is occupied by Sammy's Halal restaurant, Gifts on Sixth, Fresh Pizza, and Diana hair & nail salon, with Dekco clothing in the adjoining south section.
