Chartered in 1873, the City & Suburban Telegraph and Telephone Exchange opened its first exchange at Fourth Street and Walnut Street in 1878. In 1903 the name of the company was changed to Cincinnati & Suburban Bell Telephone.

In 1930, Cincinnati Bell constructed the present twelve story stone structure, designed by Harry Hake and featuring a second story decorative border with carved stone telephones and headphones. The building is registered on the National Register of Historic Places. The twenty story long distance switching center at 229 West Seventh Street was completed in 1975.
