5-story commercial building originally completed in the 1880s as a townhouse. Sometime after 1900, a projecting, 2-story commercial base was added. The base was renovated in 2015, and is clad in dark grey-brown stone, and the recessed upper floors in white-painted stone. The ground floor is lightly rusticated, as are the four 2nd-floor piers that project slightly to frame the end-bay single-windows. There is a wide 4-pane window in the middle of the 2nd floor; all of these windows have black metal framing and upper transoms. The main entrance is at the middle of the ground floor, with a glass-and-metal double-door and short transom below a rounded, green canvas canopy extending out over the sidewalk. This entrance is slightly recessed and bordered by two windows. Both ends of the ground floor have a secondary entrance with a glass door and stone lintel. The base is topped by a metal railing.

The upper floors have three bays of single-windows in thin, simple surrounds. A simple parapet tops the facade, with a metal coping.

Prior to housing the Crestwood Funeral Home, the building was the Buckley Funeral Home.
