This is a full directional T interchange with a three level stack. It was originally built around 1961 with I-81 as a partial Y interchange with a left exit, only connecting to the north on I-81. It was reconstructed around 1987 to realign southbound I-81, complete the interchange, and eliminate the left exit. It is located near Bristol, Virginia and serves as the northern terminus of the very short I-381.
