On April 26, 1986, at 1:23:50 AM, during a planned test of turbine generator No. 8 at Unit 4, an explosion occurred that completely destroyed the reactor. The reactor building and the machine hall roof partially collapsed. More than 30 fire spots appeared in various rooms and on the roof. As a result of the accident, a release of radioactive substances occurred into the environment, with various estimates suggesting up to 14×10¹⁸ Bq, which is approximately 380 million curies of radioactive materials, including isotopes of uranium, plutonium, iodine-131, cesium-134, cesium-137, and strontium-90.
To mitigate the consequences of the accident, the Soviet Council of Ministers established a government commission. Most of the work to eliminate the consequences of the accident was carried out in 1986-87, involving about 240,000 people. The total number of liquidators (including subsequent years) was about 600,000.
A concrete "sarcophagus" (the so-called "Shelter" object) was built around Unit 4. Since it was decided to start up Units 1, 2, and 3 of the station, radioactive debris scattered across the plant's territory and on the machine hall roof was removed into the sarcophagus or encased in concrete. The construction of the sarcophagus was completed in November 1986.
