The hospital sits on the site of Blackwell's Island Penitentiary, constructed in 1832 to relieve overcrowding. The prison was over 600 feet long and built using gneiss quarried on the island.

Some of its more famous inmates included Mae West, who spent eight days here for her role in the 1926 hit play "Sex", and Boss Tweed of Tammany Hall fame. The prison was eventually torn down in 1935, and the inmates were moved to Rikers Island.
