4-story concert hall and commercial building completed in 1945. It was originally a Polish-American meeting hall, and now houses a concert hall on the ground floor. The facades are faced in smooth stucco, mostly ruddy red, but with an area of white on the 2nd-3rd floors at the east facade, and a section at the west end of the south facade that is unpainted and set back slightly from the rest of the facade, with a narrow angled junction.
The ground floor along the south facade is of cobbled rough stone, except for the east end, which has wooden boards. The east facade has more wood planking at the south end of the ground floor, and black stone and metal across the rest, with a central entrance with two sets of double-doors below a prominent red-and-gold marquee. The 2nd & 3rd floors have two single-windows near the south end, while the 4th floor has nine single-windows placed evenly across the facade, which is topped by a stone band course.
Near the middle of the ground floor on the south facade is a round-arched entrance with red wooden double-doors framed in stone. To the immediate right is a small pass-through counter window, and to the left are two stone-framed rectangular windows and a T-shaped stone-framed opening in the west end section that contains windows, and another door at the far west end in a surround of polished red granite. Above it is a stone-framed panel with lettering reading "Polish Army Veterans Association of America". There are two single-windows next to it, and three bays of single-windows on the top two floors of the west end section. The 2nd & 3rd floors on the rest of the south facade have five bays of single-windows, (the eastern two placed closer together), while the top floor has eight single-windows.
Besides the concert venue, the ground floor is occupied by The High Note tavern.
