Former town hall for Brookhaven until 1986, when it moved to Medford and then its current location near Bald Hill in Farmingville. The facility underwent a $4.7 million, multiyear renovation and Northwell worked with the Town of Brookhaven to preserve the historical value of the building, which was constructed in 1926 to a design reportedly by John V. Van Pelt and had additions as part of a Works Progress Administration (WPA) project in 1939. The building was in use by the town until 2004. Among the historic treasures preserved: a marble, spiral staircase and telephone booth.

It is now used as a medical practice and urgent care facility for Northwell Health.
