Established in 1949 as the US Navy's Middle East Force on the grounds of a Royal Navy installation at Bahrain, the US Navy formally began its operations at Manama following Bahrain's independence in 1971 when it took over significant portions of the former British facility. Presently the headquarters of the US 5th Fleet following its 1995 recommissioning, Naval Support Activity (NSA) Bahrain now supports the operations of afloat units that rotationally deploy to the 5th Fleet from the Pacific and Atlantic Fleets, surge from the United States and squadrons of smaller ships that are based in the Persian Gulf for longer periods.

The 5th Fleet's area of responsibility encompasses about 2.5 million square miles of water area and includes the Persian Gulf, Red Sea, Gulf of Oman and parts of the Indian Ocean. This expanse, comprised of 20 countries, includes three critical choke points at the Strait of Hormuz, the Suez Canal and the Strait of Bab al Mandeb at the southern tip of Yemen.
