A single-span multiple Kingpost truss 67' over the Middle Fork of Little Beaver Creek.
The structure rests on concrete abutments. It is covered with faded gray weathered vertical boards on the sides and portals. Open under the eaves for ventilation. The deck consist of lengthwise planking and it has a shake roof. There is a advertisement painted on the portal indicating "Buy Your Drugs Of(f) C. R. Phillips."

Also known as the Centennial Covered Bridge.
