Plaza Roma was called Plaza de Armas when it was used as a venue for bullfights. The name changed to Plaza Mayor when it was turned into a garden. During the American period, the plaza was named Plaza McKinley, in honor of a US president. When Rufino Santos became the first Filipino cardinal, the plaza was renamed Plaza de Roma, and to return the honor, Rome also named one of their plazas as Piazza Manila.

In the center of Plaza Roma stands a bronze image of King Carlos IV. This statue was cast at the maestranza or royal foundry (located at Fort Santiago) to honor the king for the smallpox vaccine he has sent to the Philippines that saved many lives.
