Foreigners go to 2nd floor, counter 21 to begin. You must have: Passport; 1 photocopy each of: a) passport data page, b) visa page, c) departure card, d) front and back of current [foreign] driver's license; original and photocopy of medical certificate from your Thai doctor; and the original and photocopy of your Residence Certificate [from the Immigration Office near the airport]. The cost in April 2010 is 155 baht for car (involves computer exam), and 205 baht for motorcycle (including cycle road exam).
